fa مسمومیت با قرص برنج: تأثیر همودیالیز بر اصلاح اسیدوز و پیامد بیماران Rice pill poisoning: effect of hemodialysis on correction of acidosis and outcome of patients عمومى General گزارش مورد case report <div style="text-align: justify;">استفاده از فسفید آلومینیوم جهت نگهداری برنج و سایر غلات در انبارها و جلوگیری از تأثیر مخرب حشرات موذی، نسبتاً رایج می باشد. مصرف نادرست این سموم در انسان باعث بروز انواع مسمومیت های حاد و مزمن می گردد. در این مطالعه مسمومیت حاد با سم فسفید آلومینیوم در چهار مورد اقدام به خودکشی که تحت همودیالیز قرار گرفته و بهبود یافتند، مورد بررسی قرار گرفتند. بیمار اول آقایی 16 ساله، یک ساعت قبل از مراجعه اقدام به خودکشی با مصرف یک عدد قرص فسفید آلومینیوم کرده بود که دو نوبت دیالیز دوساعته در دو روز متوالی برای بیمار انجام شد بیمار بهبود پیدا کرده و ترخیص شد. بیمار دوم آقایی 22 ساله، یک ساعت قبل از مراجعه سه عدد قرص فسفید آلومینیوم مصرف کرده بود، جهت اصلاح اسیدوز برای بیمار3 ساعت دیالیز انجام شد. بیمار سوم آقایی 16 ساله که نصف قرص فسفید آلومینیوم مصرف کرده بود، به مدت 3 ساعت دیالیز انجام شد و با وضعیت پایدار به بخش روان پزشکی ارجاع شد. بیمار چهارم خانم 24 ساله، یک ساعت قبل از مراجعه 2.5 قرص فسفید آلومینیوم با آب کرده بود. برای بیمار 3 ساعت دیالیز انجام شد و بیمار با بهبودی کامل ترخیص شد. نتایج نشان داد که همودیالیز باعث اصلاح اسیدوز متابولیک و بهبود پیامدهای بالینی مرتبط با مسمومیت با فسفید آلومینیوم  می شود. لذا با توجه به عدم تعادل همودینامیکی و وجود افت فشارخون و شوک در اکثر بیماران مسمومیت با قرص برنج، بهتر است همودیالیز شریانی وریدی مداوم مانند استفاده شود.  </div> <div style="text-align: justify;">It is relatively common to use aluminum phosphide to store rice and other grains in warehouses and prevent the harmful effects of pests. Improper use of these poisons in humans causes various acute and chronic poisonings. In this study, acute poisoning with aluminum phosphide poison was investigated in four cases of attempted suicide who underwent hemodialysis and recovered. The first patient, a 16-year-old man, had committed suicide by consuming one aluminum phosphide tablet one hour before the visit, and two two-hour dialysis sessions were performed for the patient on two consecutive days. The patient recovered and was discharged. The second patient, a 22-year-old man, had taken three aluminum phosphide tablets an hour before his visit, and he underwent dialysis for 3 hours to correct his acidosis. The third patient, a 16-year-old man who had taken half of aluminum phosphide tablets, underwent dialysis for 3 hours and was referred to the psychiatric department in a stable condition. The fourth patient, a 24-year-old woman, had taken 2.5 tablets of aluminum phosphide with water one hour before visiting. Dialysis was performed for 3 hours and the patient was discharged with full recovery. The results showed that hemodialysis corrects metabolic acidosis and improves clinical outcomes associated with aluminum phosphide poisoning.
