fa چالش های آموزش بالینی جراحی و مراقبت‌های آن در رشته پرستاری اتاق عمل در پاندمی کوید-19 Challenges of clinical training of surgery and its care in the field of operating room nursing in the covid-19 pandemic تخصصي Special پژوهشی كاربردي Applied research <div style="text-align: justify;">vvمراکز آموزشی پس از شیوع کووید-19 بسته شده  (1)و آموزش بالینی نیز نیاز به حضور در مراکز بهداشتی و یادگیری مهارت های عملی در بالین دارد و کسانی که این اموزش هارا دریافت می کنند در آینده به عنوان یک نیروی کار در بیمارستان ها حضور خواهند داشت (2). تعطیلی مراکز دانشگاهی در کنار ایجاد چالش برای سیستم بهداشتی، آموزش پزشکی و درمان را نیز تحت تاثیر قرار داده است (2). در حوزه آموزش جراحی ، کاهش میزان مواجهه بالینی و حجم موارد درمانی ، کیفیت آموزش جراحی را به خطر می اندازد (3). به دلیل سرعت بالای انتقال این بیماری، از حضور دانشجویان به عنوان نیروی فعال در بالین جلوگیری می شود. کمبود کیس های آزمایشی و تجهیزات حفظت فردی، لغو شدن جراحی های غیرضروری و الکتیو از دیگر مشکلاتی هستند که بر سر راه دانشجویان علوم پزشکی قرار دارد (2). برنامه های طراحی جایگزین های یادگیری مجازی برای حفظ برنامه های درسی خود ، آینده آموزش جراحی را شکل می دهند (4). علی رغم تعدد منابع آموزشی انلاین، عدم حضور دانشجویان در بالین بیمار باعث می شود دانشجویان نتوانند ارتباط و مشارکت با بیمار ،که جزو مهارت های اصلی برای کادر درمان می باشد و همچنین مهارت های معاینات فیزیکی و ارزیابی بالینی را یاد بگیرند (2). همچنین تعطیلی بسیاری از کنفرانس های جراحی نیز در روند رشد وآموزش روش های نوین جراحی در اتاق عمل تاثیر منفی می گذارد (5). از طرفی کاهش سرعت و یا توقف انجام پروژه های تحقیقاتی توسط دانشجویان به دلیل شیوع ویروس کووید-19 و عدم حضور دانشجویان در دانشگاه و محیط های آکادمیک از دیگر چالش های آموزشی می باشد (5). عدم حضور در بالین باعث افزایش استرس و اضطراب در دانشجویان می شود چراکه آن‌ها درباره آموخته ها و عملکرد خودشان مطمئن نیستند و همچنین سایر عواقب ناشی از کرونا در زندگی اجتماعی، این استرس را شدت می بخشد (6). در رابطه با چالش های آموزش بالینی در رشته پرستاری اتاق عمل می توان به افزایش میزان استرس دانشجویان، کمبود کیس های جراحی متنوع، احتمال انتقال کووید-19 به خانواده های دانشجویان، دور بودن از فیلد های جراحی، تراکم جمعیتی دانشجویان در مقاطع زمانی کوتاه، کنسل شدن تعداد زیادی از اعمال جراحی و عدم مشاهده تمام اعمال جراحی در دوره آموزش خود، کمبود وسایل حفاظت شخصی و تجمع دانشجویان در خوابگاه ها و اتاق های آموزش بالینی محدود اشاره کرد. پیشنهاد می شود که حتی الامکان از تعداد دانشجویان کمتری در آموزش بالینی استفاده شده و استفاده از شبیه ساز های جراحی به آموزش این رشته اضافه گردد. همچنین با واکسیناسیون کامل دانشجویان و تامین وسایل حفاظت شخصی، در محیط های بالینی حضور یابند. علاوه بر این باید سطح دانش و آگاهی دانشجویان افزایش یافته تا بتوانند علاوه بر آموزش، از آلودگی کووید-19 نیز پیشگیری کنند.</div> <div style="text-align: justify;">Educational centers have been closed after the outbreak of Covid-19 (1) and clinical training also requires attending health centers and learning practical skills at the bedside, and those who receive these trainings will be a workforce in hospitals in the future. will be present (2). Besides creating a challenge for the health system, the closure of academic centers has also affected medical education and treatment (2). In the field of surgical training, reducing the amount of clinical exposure and volume of treatment cases endangers the quality of surgical training (3). Due to the high speed of transmission of this disease, the presence of students as an active force in the hospital is prevented. The lack of test cases and personal protective equipment, the cancellation of unnecessary and elective surgeries are other problems facing medical students (2). Programs designing virtual learning alternatives to sustain their curricula will shape the future of surgical education (4). Despite the abundance of online educational resources, the lack of presence of students at the patient's bedside makes students unable to communicate and participate with the patient, which is one of the main skills for the treatment staff, as well as learn the skills of physical examination and clinical evaluation (2). Also, the closure of many surgical conferences has a negative impact on the development and training of new surgical methods in the operating room (5). On the other hand, slowing down or stopping research projects by students due to the spread of the Covid-19 virus and the absence of students in universities and academic environments are other educational challenges (5). Not being present at the bedside increases stress and anxiety in students because they are not sure about their learning and performance, and other consequences of Corona in social life intensify this stress (6). In relation to the challenges of clinical education in the field of operating room nursing, it is possible to increase the stress level of students, the lack of various surgical cases, the possibility of transmission of covid-19 to students' families, being far from surgical fields, the population density of students in short periods of time. He pointed out the cancellation of a large number of surgeries and the lack of observation of all surgeries during his training period, the lack of personal protective equipment and the gathering of students in dormitories and limited clinical training rooms. It is suggested that as little as possible, the number of students is used in clinical training and the use of surgical simulators is added to the training of this field. Also, by fully vaccinating students and providing personal protective equipment, they should be present in clinical environments.
